Yeshasvini Health Insurance Scheme was inaugurated on June 1, 2003, by former Karnataka Chief Minister S.M. Krishna. The Yeshasvini Co-operative Farmers Health Care Scheme, specifically developed for farmers who are members of state co-operative organisations, provides complete health insurance protection with 823 surgical operation coverage.
The scheme extends throughout 30 districts in Karnataka. The Yeshasvini Cooperative Farmers Health Care Trust, registered under the Indian Trust Act 1882, is entirely responsible for the scheme's operation.
What Is Covered Under Yashasvini Health Insurance Scheme
The Yeshasvini Healthcare Insurance Scheme pays for the following surgical procedures:
1) Cardiothoracic Surgery
2) Cardiac Arrest
3) Vascular, Neurological, Ophthalmology, Genito-urinary Surgery
4) Normal Delivery
5) Snakebite
6) General Surgery
7) Dog bite
8) Accidents while doing agricultural activities
9) Paediatric, Orthopaedic Surgery
10) Obstetrics
11) Gynaecology Surgery
What Is Not Covered Under Yashasvini Health Insurance Scheme
The Yeshasvini Healthcare Insurance Scheme excluded the following surgical procedures:

3) Skin Treatments, Cosmetic Surgery, Grafting
4) Kidney transplant
5) Burns
6) Regular Check-Ups
7) Chemotherapy
8) Joint Replacement Surgery
9) Dialysis
10) Autoimmune Diseases
11) Inpatient Medical Treatment
12) Road Accidents
13) Diagnostics Tests
14) Nasal Septum
15) Vaccination

The following documents are required for the Yashasvini Health Insurance Scheme:
1) Address Proof
2) Age Proof
3) A certificate proving a person's membership in rural or urban cooperative societies.
Features and Benefits of the Yeshasvini Health Insurance Scheme
The Yeshasvini Health Insurance Scheme has the following essential features and benefits:
1) Farmers who have been members of a state cooperative organisation for at least three months are the primary beneficiaries of this scheme.
2) No age limit is prescribed. The beneficiary can take treatment from newborn to his/her lifetime.
3) The policy provides insurance coverage for all of the mentioned ailments.
4) The Yeshasvini Health Insurance Scheme covers 823 surgical operations divided into standard delivery, Neonatal Care, and Angioplasty.
5) A rural Yeshasvini member can get free medical operations worth Rs 1.25 lakh and Rs 2 lakh. In comparison, an urban member can get treatment worth Rs 1.75 lakh for a single hospitalisation and Rs 2.50 lakh for several hospitalisations.
How to Claim Yeshasvini Health Insurance Scheme
All claims must be submitted within ten days after the Beneficiary's hospital discharge. Where credit bills are not accessible, you may submit a bill under the letterhead of your hospital as a "credit bill" with the bill number and IP number. The claims must be supported by a checklist that ensures the presence of the following documents: -
1) Attested Photocopy of the Yeshasvini I.D. Card.
2) Photocopy of Aadhar card
3) Voter ID/DL/Ration Card
4) Credit Final bill – raised in favour of “Yeshasvini Trust with the beneficiary name”.
The exact amount should be claimed in the final bill provided
5) Original claim form duly signed by the beneficiary
6) Original discharge summary.
7) Original Operation Notes.
8) In case of an Angioplasty procedure, please enclose the Angiogram CD and report.
9) All Laboratory/Investigation reports that are attached to E-Preauth to justify
the diagnosis.
10) In case of enhancement taken in Preauth, both initial & enhancement
authorization letters have to be attached as part of the claim.
11) All preauthorization approval letters must be duly signed by our District
Coordinator before claim submission.
12) All documents submitted during preauthorization should be part of the claim
submitted.
The Network Hospital will be reimbursed from the Yeshasvini Co-operative Members
Health Care Trust (a Trust formed by the Dept. of Co-operation, Govt., of Karnataka)
as per the standardized and Pre-defined Package Rates.
How to Get a Yeshasvini Health Card?
Yeshasvini health card is a necessary ID that allows one to access healthcare services. The scheme holder can receive necessary care under the cashless category by showing this card in a hospital. This card is given to you when you purchase the scheme and is renewable at all corporate societies. Yeshasvini card application online registration can be started at http://sahakara.kar.gov.in/.
How to Register for the Karnataka Yeshasvini Health Scheme?
To be eligible for this scheme, the applicant must have been a member of a cooperative society for at least three months. Every year, from May to June, all relevant corporative societies enrol new members for the Karnataka Yeshasvini Scheme.
