Fill it out, attach the required documents, and pay the prescribed fee. Officials will verify the details entered in the application. If the details match the related hospital records, the certificate is issued.
Register the child’s birth within 21 days of the date of birth. After 21 days, obtain permission from the registrar of births and deaths.
